Throughout the stay with us, you can expect the highest level of therapy services. Members of our team include a full-time physiatrist, a rehabilitation psychologist, nurses who are certified in rehabilitation care as well as physical, occupational, speech, and recreational therapists.

Admission Criteria
A patient in need of rehabilitation on an inpatient basis requires all of the following:
- 24 hour physician availability
- 24 hour rehabilitation nursing
- An intensive level of therapy including a minimum of Physical Therapy and Occupational Therapy services, 3 hours a day, 5 days a week
- Must be medically stable
How to be Screened for Admission
As a provider of acute care, we accept referrals from physicians and discharge planners, as well as directly from patients. To be screened for admission, please ask your physician or discharge planner for a referral or contact the hospital directly at 989-734-7545 and ask for the Business Office Manager. Once a referral is made, a Clinical Liaison will contact you to schedule a screening. The Liaisons can visit you at the hospital or at home depending on your need and will work with your physician and ours to make your admission as smooth as possible.
